Joburg-born Ross Vintcent will wear the No 8 jersey when Italy face the Springboks in Port Elizabeth on Saturday.
The 24-year-old loose forward came off the bench in the first Test against the Boks in Pretoria last Saturday.
Vintcent, a Bishops old boy, qualifies for Italy through his maternal grandfather and has earned 16 caps for his adopted country.
Italy were outplayed in the first half at Loftus, trailing 28-3, but produced a spirited fightback in the second to lose 42-24.
